This form is to be filled out for any player choosing to be removed from an All State Games roster based on the IHSA All Star By-Law. A player who is an incoming Freshman to a High School in Illinois, a player who DOES NOT participate in HS softball in Illinois, a 2024 graduate of an Illinois High School or a player from Missouri DO NOT COMPLETE THIS FORM!
